In blender or food processor with metal blade, combine all ingredients. Cover; blend at medium-low speed until smooth. Pour into freezer container. Freeze at least several hours before using. To serve, stir 2 level tsp of mix into 6 oz hot coffee. Mix can be stored in freezer for several months. Makes 1-2/3 cups mix.
